Abstract

With advances in medical technicology, a changing social structure, and increasing chronic and geriatric diseases, many changes have accured in health care systems, including a move toward early hospital discharge and home care nursing. Home care nursing is viewed as not only the most preferred way to provide card to clients, but also the most cost effective.
In 1989 the Korean government passed a law that opened the way for formal development of home care nursing system. Since 1994 four hospitals have provided home care nursing as demonstration project and now 45 hospitals participate in that project.
Evaluation of the home care nursing demonstration projects will be very important for the decision on making home care coverage through government insurance available on a national scale. This study was done in order to provide basic data that would contribute to the development of home care nursing system.
The subjects of the study were 304 from chronically ill patients in D iniversity hospital in P city during period from Feb. 16 to Mar. 15 1998.
The result of the study can be summarized as follows
1. The acknowledge level of the subjects to the home care nursing system that government plans to carry out was surveyed and founded as under : 70.4% responded they heard about home care nursing system. 50.9% of them get information from posters of D university hospital
2. 87.2% said that they thought the home care nursing system was "very necessary" and "somewhat, necessary.¢¥ 60.9% said they would like to use the system and they pay high attention about the quality of the home care nurses, the procedure of using the system and the fee.
3. There was a significant difference in requirement (P=.0001) among the 10 home nursing service categories. The need was highest in the category of patient assessment.
Over 50% of the clients showed needs for the following 7 home care nursing service activities 10 observation and health assessment (2 check for vital sign Q3 intravenous injection blood sampling Q5 diet teaching counseling for health condition 07 health problem finding and nursing diagnosis.
